All Angelenos 5 and older are eligi= ble to receive their vaccine at City sites. Eligible Angelenos can pre-regi= ster for a vaccination or find the nearest location. All Angelenos under the age of 18 who get vaccinated= at any City site must bring= this form signed by a parent or legal guardian. Please note that only the Pfiz= er vaccine is authorized for those 5 years and older, while the Modern= a and Johnson & Johnson vaccines are authorized for those 18 years and = older.
